Patients with poor control of chronic diseases will be identified by patient registry. Patient, physician and Hope Center therapist will meet, identify barriers to healthy life style (diet, exercise, stress management, weight) and set goals.
Patients will also be screened for depression. Therapist will see patients weekly for 8-12 weeks.
Therapists will be in the PC office 1-2 days per week. At 10-12 weeks, therapist and physician will meet to assess progress and revise treatment plan, as needed. Patients meeting goals will be offered monthly group therapy for maintenance.
IT consultant will develop EHR therapy note template. Therapist will enter note into primary care EHR and copy and paste note into email to transmit to the Aspen Hope Center patient record. Patient and primary care provider will meet every 3 months to monitor changes.
