Private primary care and private behavioral health collaborate to grow and sustain PCBH (Primary Care Behavioral Health) by expanding behavioral health availability to two physician groups and an urgent care center.
This project specifically targets how to evolve and replicate PCBH in the private arena where 67.7% of patients receive care.
The project requires the primary care physicians to increase their knowledge and utilization of PCBH by using screening tools/protocols and PCBH consultations, increased PCBH warm handoffs, increased use of joint sessions with complex patients, and increased utilization of after hour emergency referrals when psychological or psychosocial factors warrant such involvement.
This project will also see the collaborating partners moving towards a shared EHR.
