Behavioral Health — A Shared Service Model

This project plans to integrate mental health services to include acute intervention, counseling and technology-based chronic care management by imbedding behavioral health practitioners into a family medicine practice part-time.
A combination and comparison of capitation and FFS will test feasibility and sustainability of this model.
Westminster will offer services provided by an onsite behavioral health professional (BHP) to patients in need of this service. Chronic disease management will be implemented using the LiivMD multimedia health platform.
The Integrated Health Coach (IHC) will provide chronic disease management services onsite through a fee for service model that will then fund an on-site behavioral health professional after the grant ends. The BHP and IHC will document services rendered in the practice's EHR.
